You will be directed to the next page. See diagram below. Fig.02. Here you will select your passport type from the drop down menu and choose Standard Passport. Then click on Start Application. Which is right underneath this will take you to the form page proper.
In the form page, you are required to fill the blank spaces correctly with your data as you will want it appear on your passport. If you already know how to fill this form click here to skip this step to step guide on filling this form. Fill the form as follows
Title:chose the title that best suite your personality
Last name: this is your surname (your father’s name)
First name: this is your given name
Middle name: your other name e.g Michael John Efiong. Where John is the middle name
Sex: choose your sex
Date of birth: indicate your date of birth like this 2009/07/28 which is year, month and day or just click on this icon beside the box which will open a dialog box for you to simply choose from, see diagram below. Fig.03. Note: you should choose first, the year, followed by the month and then the date.
In this diagram , the two white triangles on which I placed red circular shapes for emphasis are what you will click on for the drop down options for the month and year respectively.
Place of birth: this is the town you were born
Permanent address: this is your present house address in which ever country you live in
Request type: leave the default option which is Adult except the passport is for a kid
State of origin: this strictly mean the state you came from (where your home town is in Nigeria)
Nationality: don’t touch this as Nigeria is already chosen as default.
Home town: this is your family home town which definitely is in your state of origin.
Occupation: whatever you do for a living e.g business man
Contact phone: your phone number
Email: your email address
Mobile: it can still be the same phone number you indicated above
Maiden name: this option is only for married women who now bear their husband name as their last name. In this case their real father’s name is their maiden name
We now go to personal features:
Marital status: chose if married, single, divorce or widow etc.
Colour of eyes: chose your eye colour
Colour of hair: chose your hair colour
Height (m): chose your height in metre e.g. 1.65
Special features: you can leave this space blank, except you have a special feature to add e.g. I have wings.
Next of kin: this is the name of any of your immediate relatives home or abroad. It is better you use the name of one closer to you.
Address: indicate his address (your next of kin)
Then the processing office/ Embassy –section–
Country: choose the country where you are going to process your passport e.g. Spain, and leave the rest options on the same line to take care of themselves.
Then right below is the agreement policies, make sure you check the box beside I Agree then go below and hit print form to print your form. Before you now click on submit application to go to the next page. Note: in the printed form write the date on the date space and sign on the signature space. See diagram Fig.04 below.
